Location: Sign up will be at the First United Methodist Church Grace Building at 303 East Ida Avenue.
When: Friday, November 6 during the hours of 8 am-12 pm and 4-6 pm.
Due to the COV-19 virus, masks and social distancing will be required.
For children 12 and under. Assistance for the first 100 children registered for families with Opp addresses. Must have correct sizes for shoes, jackets, cloths and diapers.
Application will be available the day of sign up. You must bring all required documents the day of sign up:
Valid driver’s license; social security card (copy); proof of income (no handwritten notes); proof of guardianship (if applicable); EBT card (if applicable); child’s social security card (copy); child’s birth certificate (copy).
Providing of documents is parent or guardian responsibility. Your documents will be checked prior to filling out an application. If you do not have the necessary documents you will have until the end of the day to provide them and may return to complete the application.
